5. For customers requesting payment terms,you will use the Wholesale Order Form, a pdf download to place orders.
This order form can be mailed, faxed or emailed.  Note that starting 9/1/2010 payment terms may not be set up for new customers
on opening orders.

WHOLESALE INFORMATION:

MINIMUM

Kits and refill items are sold by the case (6 items in a case). Prices are available to approved Wholesale Customers, please submit your wholesale account form above to us to get started!

TERMS

All new orders are pending approval and territory check. We reserve the right to approve selling on all Web sites. Retailing on eBay/Amazon stores is not allowed. See Wholesale Standard Terms for full terms & conditions. Payment terms of NET 30 are available and outlined in the New Customer Account Form.

SALES TAX

We will charge sales tax unless we receive an exemption certificate. Sales tax cannot be refunded after products have shipped.

SHIPPING

We ship UPS/Fed Ex ground. Freight options are available for orders weighing over 200lbs.

RETURNS

Prior authorization required. Credit or replacement for damaged or missing goods if reported within seven days of receipt. Goods that arrive damaged, please hold shipment for 48 hours for UPS to issue claim. See Wholesale Standard Terms for detailed return merchandise information.
